Follow these simple steps:
1. You can email to Crisis Intervention Section: ciu.fo7@dswd.gov.ph
- SWAD Bohol: swad-bohol.fo7@dswd.gov.ph
- SWAD Cebu: swadcebu.fo7@dswd.gov.ph
- SWAD Negros Oriental: swad-negor.fo7@dswd.gov.ph
- SWAD Siquijor: swad-siq.fo7@dswd.gv.ph
2. Wait for the confirmation text from the DSWD that contains the date and place where the assistance will be given.
3. Bring the requirements that are needed.

- Field Office VII at M.J. Cuenco Ave., cor. General Maxilom Ave., Barangay Carreta, Cebu City.
- SWAD Bohol at Bragy. Poblacion III, Tagbilaran City
- SWAD Negros Oriental at Solon Apartment, Barangay Bantayan, Dumaguete City.
- SWAD Siquijor at Brgy. Tacdog, Siquijor (at the back of Jollibee).
