The 37-year-old Architect Michael Alfonso falls down to death on Saturday night after he mistakenly enters a wrong door which seems to be an ordinary door to a toilet.
Report said at about 9 pm, rushing Alfonso opened the door, entered and fell through the elevator shaft from the second floor down to the basement and onto the top of the elevator box, the report said.
Alfonso who came from a gym, hurriedly searched for a comfort room in the building where the incident happened.
Initial investigation appeared that there were no safety warning signs in the area and the door is looked like an ordinary door to a CR.
